Actually what I think they are useful for is showing that 15kHz is not something that matters a big deal when it comes to music. I think if they didn't know most people would hear a 1kHz tone and expect it to be more like 10kHz

Yes, if not using feedback, just combine the resistors and take the cap to ground.

No, a ultrapath cap is just between B+ and the cathode. And of course at AC B+ is effectivly the same as gnd. But the point of the ultrapath cap connection is the signal current loop doesn't go via the last cap in the power supply.
